Youth Information

Please fill out the information below for your child attending our Youth program. If you have more than one child attending, please complete a separate form for each child.

I give permission to the staff and volunteers of Sherwood Park Alliance Church Society to take photographs and video of myself and/or my minor children ("Photographs and Video"). I understand that these Photographs and Video may be used for the sole purpose of future promotion of the activities of Sherwood Park Alliance Church Society, through the posting of the Photographs and Videos on the Sherwood Park Alliance Church Society website and/or social media platforms and in related print and/or digital promotional materials.

Personal information is collected in accordance with the Personal Information Protection Act. By filling out this form, you authorize Sherwood Park Alliance Church to collect your information and your child's information for ministry purposes. Information will be entered into our database that currently resides in Canada. If you have any questions or concerns about any collection, use or disclosure of personal information by Sherwood Park Alliance Church, please contact our Privacy Officer.
